Payment of registration fees All fees for congress registration and social events should be paid in EURO to ‘Stichting EACS’, clearly stating the participant’s name and address. Payment can be effected either by bank transfer (free of charge) to ING Amsterdam, Account No. 65 80 48 155 (swift code: INGBNL2A) or by credit card (American Express, Visa, Eurocard/Mastercard) or by bank cheque (to be sent to the Congress Secretariat). Confirmation Upon receipt of the correct registration fee, each participant will receive a confirmation of registration. Please bring this confirmation to the registration desk at the Okura Hotel as proof of your registration. Cancellation / Refund policy Refund of registration fees and social events, less 25% administrative charges, can be applied for in writing until 30 April, 2003 to the Congress Secretariat in Amsterdam. After this date no refund will be possible. For any change of names, a fee of EUR 25 will be charged. On-site changes of name badges due to incorrect submission of names and/or address data will be charged with EUR 10. Hotel accommodation Limited accommodation is available at the Okura Hotel, Amsterdam. All requests will be handled on a ‘first come-first served basis’. Hotel rates and deposit Okura single: Standard Room: EUR 225; Corner Room: EUR 260 Payment and cancellation policy for hotel rooms Hotel accommodation invoices are settled upon checkout. In order to make and secure a reservation a creditcard guarantee of EUR 225 must be given. Cancellations of hotel accommodation will be accepted in writing only until 23 May, 2003. In case participants cancel after 23 May or in case of ‘no show’ the guarantee of EUR 225 will be charged. Any change of reservation will be subject to a handling fee of EUR 25. Please use the enclosed Hotel Reservation Form by completing it with your credit card details.

Nevertheless a Currency The official congress currency is Euro (EUR / €) Dinner This promises to be an extraordinary and exciting evening with the opportunity to see some of the delights that Amsterdam has to offer. The dinner on Saturday, 31 May is included in the registration fee for participants and accompanying persons. Separate dinner tickets can be bought at EUR 125. Insurance The congress organisers cannot accept liability for personal injuries sustained, or for loss or damage of property belonging to congress participants (or their accompanying persons), either during, or as a result of the congress. Please check the validity of your own insurance. Language The congress language is English. Simultaneous translation will not be provided. Letter of invitation The organisers will be pleased to send a letter of invitation to any individual requesting one. It is to be understood that such an invitation is intended to help potential participants raise travel funds or obtain a visa. It is not a commitment on the part of the congress to provide any financial support. You may request such a letter from the Congress Secretariat.
